CHCs Provide Health Care for the Homeless

On December 18, 2013, Colorado Coalition for the Homeless, one of Colorado’s Community Health Centers (CHCs), held the twenty-fourth annual homeless person’s vigil. The vigil honored individuals who had lived and died on the streets of Denver in 2013. Over 100 names were read, which shows why affordable, accessible, and high quality health care services for people experiencing homelessness are so essential.

While high health care costs and debt related to medical expenses can be a contributing cause to homelessness, homelessness also exacerbates health conditions. Individuals experiencing homelessness face many barriers to accessing the care they need such as affordability, transportation, and availability of services, especially mental health and substance abuse treatment. Without a stable place to live, chronic diseases such as diabetes and hypertension are extremely difficult to manage. People living on the streets are more likely to be victims of violence and to be exposed to harsh weather conditions. Healing from wounds or illnesses is much harder without a safe, warm place to do so. It becomes very difficult to prioritize health needs when finding shelter, food, and safety are consistent struggles.

While people experiencing homelessness face many health access barriers, CHCs throughout Colorado are helping to meet these complex needs. All CHCs provide affordable, culturally competent services to vulnerable populations in their communities, and some—like the Colorado Coalition for the Homeless—provide services targeted at the homeless population. Besides having accessible locations and sliding fee scales for individuals who do not have insurance or incomes, CHCs provide comprehensive services so that their patients can access not only medical but also dental, vision, and behavioral health services. CHCs also offer enabling services such as case management and health education, which provides additional support to vulnerable populations. CHCs are having an important impact on people experiencing homelessness in their communities!

Improving Access to Affordable Health Care – Colorado Community Health Centers’ Mission

The Colorado Health Institute (CHI) released their 2013 Colorado Health Access Survey (CHAS) last Wednesday, Nov. 5. The report provides statistics regarding barriers to receiving health care, uninsured and underinsured measures, utilization rates, and health status for Coloradans. As seen in the CHAS, “one in seven residents or 14.3 percent of Coloradans do not have insurance.” The survey also reports that 16.9 percent have “no usual place of care” compared to insured populations, 83.1 percent of which have a “usual source of care.”

Community Health Centers (CHCs) strategically help combat access barriers and provide a medical home for uninsured and underinsured Coloradans. CHCs do this by thinking about the communities and vulnerable populations they serve. They expand access through building renovation, increasing hours and services, developing more clinic space, and planning for growth opportunities. Services are targeted to families earning less than 200 percent of the federal poverty level.  Services are offered on a sliding scale fee based on income to keep care affordable.

In the past two years, CHCs have been making strides to increase access to care statewide, including:

  • Four organizations have been designated as CHCs since June of 2012:
    • Axis Health Systems, Durango, operating a site providing primary and mental health care.
    • Olathe Community Clinic, Inc., Olathe, operating a site providing primary and mental health care.
    • Sheridan Health Services, Sheridan, two new clinic sites –a community clinic and a school-based health center (SBHC).
    • Summit Community Care Clinic, Frisco, operating multiple clinic sites, including four SBHCs in Silverthorne, Dillon, Breckenridge and Frisco.
  • In 2013, new building expansion and renovation have increased services:
    • Metro Community Provider Network opened a new facility in Wheat Ridge, Elmira Refugee Health Center in Aurora, and reopened the Helena Clinic in Aurora which was MCPN’s very first original site. Additional clinic sites will open this year that include the Pine Tree Health Center in Parker and Health and Wellness Center at the Asian Pacific Development Center.
    • Plains Medical Center, Inc. renovated and reopened their Limon Clinic featuring a new dental suite and physical therapy services in additional to primary and behavioral health care.
    • Northwest Colorado VNA – Community Health Center opened a new site in Steamboat.

This is small portion of CHC expansion, development and improvement activities that aim to reduce access barriers for Colorado’s vulnerable populations. October 11, 2013, health professionals, Community Health Centers (CHCs), partners and students celebrated National Primary Care Week and Corps Community Day.  Colorado Community Health Network (CCHN), Community Health Association of the Mountain/Plains States (CHAMPS), National Health Service Corps (NHSC), Colorado Health Service Corps (CHSC), and Colorado Rural Health Center hosted a local Corps Community Day Connection and Awards Ceremony event on Oct. 11, 2013.  The purpose of our Corps Community Connection and Awards ceremony was to celebrate the clinicians who have promoted the importance of primary health care by working in clinics in rural and underserved communities in Colorado.

Mission-driven: Eighteen CHCs and the safety net provide primary health care for one million people in Colorado. CHCs care for lower income people in many of Colorado’s communities. It is known beyond a doubt that CHCs are doing something incredibly important, and CHC staff, patients and partners see the impact made every single day.

The future of health care: CHCs are on the forefront of outcomes-based medicine in a setting that is digitized, integrated and forward-looking.

Meaningful workplaces: CHCs are rewarding, collaborative, innovative places to work. There is something energizing about being part of team doing important work in a creative environment.

Colorado CHCs:

  • Employ health care professionals with a mission to provide comprehensive primary medical, dental and behavioral health care to low-income working families.
  • Provide a health care home to more than 600,000 of their community members – one in 10 people in the state.
  • Participate in innovative quality improvement programs to build health care homes, improve childhood immunization rates, control diabetes and hypertension, ensure women’s health improvement, and better the public health of Colorado communities.

On August 1-3, CCHN staff exhibited at the American Academy of Family Physicians conference in Kansas City, Missouri to raise awareness about Colorado Community Health Centers (CHCs). Over 4,000 family medicine medical students, residents, leaders and educators participated in the three-day conference.

During the conference participants attended meetings and education sessions that focused learning on leadership, research, health policy, clinical skills and career planning.  At the National Congress of Family Medicine Residents forum, residents discussed issues and created resolutions regarding substance and alcohol abuse, over the counter contraception, federal drug registries, obstetric ultrasound screenings by family physicians and more.

CCHN is excited to see the passion these residents and students have for a more educated and healthier public. Additionally, the outpouring of conference attendance is encouraging because it shows that there are a number of health professionals interested in family medicine. CHCs have a strong need for high quality family medicine physicians with a mission to provide health care for low-income working families across Colorado communities.

CHCs in Colorado employ 220 family physicians along with 153 other physicians ranging in disciplines from internists, to specialists and pediatricians (based on health center data from the Bureau of Primary Health Care’s 2012 Uniform Data System). According to the AAFP website, “family physicians less than seven years out of residency earn (on average) more than $145,000.”  CHCs also provide comparable salaries; on average family physicians earn $150,859 according to the CHC 2012 Salary Benefits and Turnover Survey.  CHCs offer satisfying employment by offering robust benefits packages, high quality health care, greater scope of practice, relationships and teamwork with other employees, and a mission driven setting.
