High Plains Celebrates National Women’s Health Week

This week, May 13 – 19, 2012, is National Women’s Health Week. A spotlight on women’s health, this week encourages women to make their health a top priority through regular check-ups, being active and eating healthfully, paying attention to mental health, and avoiding unhealthy behaviors.

Colorado’s Community Health Centers (CHC) are among those celebrating National Women’s Health Week. One of Colorado’s rural CHCs, High Plains Community Health Center (HPCHC), is launching special programs and community outreach.

Staff at HPCHC have set aside special times each day to see women in need of care. These special times are open for any kind of service a woman may need — from a wellness exam to addressing an acute illness.

HPCHC doesn’t stop their special health outreach after Women’s Health Week, however. Their community classes and events help their patients – women, men and children – in Lamar and surrounding areas learn more about eating right, being physically active, managing diabetes, and leading a healthy life year-round.

Every Saturday morning, for instance, community members meet for the Saturday Stroll. Families and individuals walk or run around a local park, burn some calories, and enjoy the company of neighbors. Community members can also get support in losing weight with Get Lean Lamar! meetings on Thursday nights.

With these and other programs, HPCHC exemplifies the creative spirit and community efforts found in Colorado’s CHCs.

The National Farmworker Health Conference May 9-11, 2012

Colorado Community Health Network (CCHN) proudly supports the National Farmworker Health Conference occurring in Denver, Colorado on May 9-11, 2012. The conference is sponsored by the National Association of Community Health Centers (NACHC) who advocates for health care needs on the behalf of 20 million people served by Community Health Centers (CHC) located in all 50 states and U.S. territories.

Fifty years ago in 1962 President Kennedy signed the Migrant Health Act into law which established the Migrant Health Program. The Migrant Health Program addresses the basic medical needs of farmworkers. Migrant and seasonal farmworkers are a critical part of Colorado’s multi-billion agricultural industry. Farmworkers face health care concerns and uncommon safety risks related to travel, farm equipment, and pesticides. Colorado’s Governor John Hickenlooper proclaimed May 9-11 in honor of Farmworker Health.

Colorado CHCs provide affordable, high quality, primary health care to more than 11,000 hardworking migrant and seasonal farmworkers.  CHC services are located in regions throughout the state at High Plains Community Health Center in the Southeast Plains region; at Pueblo Community Health Center, Inc., in the Pueblo region; at Salud Family Health Centers in Metro Denver, Northern Front Range, Boulder/Broomfield, and Northeast Plains regions; at Sunrise Community Health in Northern Front Range and Northeast Plains regions; and at Valley-Wide Health Systems, Inc., in the San Luis Valley, Western Slope, Central Mountains, Southwest, Arkansas Valley, Northeast Plains, and Southeast Plains regions.

The Colorado Coalition for the Homeless seeks an RN and Health Center Coordinator of Daily Operations to manage the daily clinical care and nursing activities in the clinic and is directly responsible for the supervision of the Medical Assistant staff. This position reports to the Manager of Nursing Services and is located at the West End Health Center, Denver, CO

The West End Health Center is a satellite of Stout Street Clinic which was established as part of the Health Care for the Homeless Project. This project was established to provide medical, mental health, dental care, substance treatment services, outreach, and other supportive services to homeless and at-risk individuals and families.

Primary Responsibilities
1. Provides skilled nursing care according to accepted community standards of care in order to preserve or enhance the patient’s highest level of functioning.
2. Gathers information for the patient data base including:
• Complete medical and psychiatric history
• List of current medications, dosages, and allergies.
• Vital signs, height, weight, and head circumference as indicated.
• A brief statement of the patient’s chief complaint.
• Pertinent information such as laboratory and/or test results.

Care Manager – 1300

Job Description:
The Care Manager is responsible for coordinating the care of SALUD patients that are stratified as “high risk”, with emphasis on those enrolled in the Accountable Care Collaborative (ACC). The aim is to improve patient health and reduce health care costs. Their primary role will be developing comprehensive care plans that address the medical and social needs of each patient.

The Care Manager will work closely with SALUD patients, SALUD clinics, local community hospitals, community members and partnering agencies. The Care Manager will work in a team-based approach, interacting with various staff members across departments, as well as supporting patients and their families to facilitate self-management, self-efficacy, and behavior change. The Colorado Coalition for the Homeless seeks a Psychiatric Nurse to provide direct nursing services and mental health clinical assessment and treatment for Bridges clients in collaboration with a multi-disciplinary treatment team. The position reports to the Bridges Program Manager and Bridges Psychiatrist. The position is located at 2111 Champa, Denver.

The Bridges High Intensity Treatment Team (HITT) works collaboratively with public and private agencies to respond to the service needs of individuals who are homeless and experiencing serious and persistent mental illness.

Primary Responsibilities
1. Assesses and monitors clients’ ongoing psychiatric, medical and social needs.
2. Oversees medication monitoring and medication administration under the supervision of the Bridges psychiatrist, or RXN/CNS.
3. Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment; acts as a psychosocial educational resource by assisting clients in learning about their mental illness, medications and how to protect their overall health.

The Medical Director collaborates with other members of the integrated health, behavioral health, nursing and support services leadership team to provide exceptional integrated care. The position is based at the Coalition’s Stout Street Clinic in downtown Denver (2100 Broadway, Denver, CO 80205) and reports to the Director of Health Services.
